Log your volunteer hours using the forms below!
We are tracking volunteer efforts in two ways:
Community based volunteering involves being engaged with multiple residents from one or more properties.
One-to-one companionship involves providing companionship to one person, either a PHC resident or someone who does not live at a PHC property (a non-resident).
We ask that you track the number of shifts and the number of hours volunteered, by month, in the above categories and use the appropriate form below to submit. If you have any questions or trouble please contact us at volunteer@plyhc.org
